Colour 3D-Reconstructed Computed Tomography Angiography Delineates Blood Flow Through Aortocaval Fistula Pre- and Post-EVAR

A 58 year old man was admitted with atrial fibrillation and lower limb swelling without abdominal pain. Computed tomography angiography (CTA) showed an isolated aortocaval fistula (ACF) secondary to aortic aneurysm (85 mm) rupture. The patient underwent urgent Gore C3 Excluder (W. L. & Associates, Flagstaff, AZ, USA) endograft placement immediate symptom resolution. Post-processing imaging software (Global Illumination Rendering v7.0; Vital Images, Canon Medical Systems, Paris, France; 2000 × pixels/300 dpi) allowed three dimensional reconstruction of blood flow on pre-operative CTA (A), later identifying a small ACF fed by type II endoleak (B), noted have resolved the two month surveillance CTA.Figure 1
